Profile:

 

The laboratory for the local health center in the town of Balete is a much-needed facility to help improve health services to the community. The laboratory will be operated and managed by Dr. Fred Villaruel who is the local physician and director of the health center for over 20 years. His vision is to have a small laboratory to provide basic clinical testing for the indigent patients who cannot afford to have their tests done in Kalibo. Although the patients will be charged minimally for the tests, Dr. Fred is confident that it will be a self-sufficient project.

 

Fund Disbursement Plan:                     

 

January 2002                30%                      $1,500.00

 

After careful review of the plan, blueprint and financial proposal, Balete Association International has agreed to release full funding for the project through income raised from the specific laboratory fund raising that started last year in October and on going.

 

Standard mobilization funding of 30% of cost to be released to Dr. Fred Villaruel as Chairman of the project in Balete and will be responsible to oversee, manage and submit Project Status Report to the Mike Calizo on behalf of the association. Details of expenditure and timeline of the project is important and necessary to authorize release of the balance of the funding.

 

March 2002                30%                      $1,500.00

 

Upon review of the Project Status Report, second release of funding will be submitted to the association for approval. Barring any unforeseen problems and delays, this schedule should stand firm due to the simple nature and size of the construction.

 

April 2002                40%                      $2,000.00

 

Balance of the budget to be released based on a complete review of the project and determination of completion date on schedule and hopefully with in planned expenditure.

 

 

Rationale:

 

This summary is issued to establish a formal schedule for the project and to distribute information to Balete Association Members. It is important to note that Dr. Fred Villaruel, as Chairman of the project in Balete will initiate and update the Project Status Report. The report will be furnished to members as necessary for their information, for presentation at association meetings and for appropriate accounting purposes.
